Referral Process

The 5 step process is robust and thorough, ensuring that our professional team are able to assess the individual and their personal needs, determine the appropriate pathway and identify a high-quality person-centred approach to deliver their support needs.

Submitting a referral
A referral can be submitted by;
- emailing the team at referrals@stmartinoftours.org.uk who will send you a referral form to complete and return or complete this over the phone with you.
- Or you can fill in our online referral form at making a referral online
Our team will review the initial documentation and respond within 48hours to discuss the referral and request further information that may be required.
On receipt of all necessary information, we will review suitability of the referral and provide a date for a face to face assessment within 48 hours.

Face-to-face Assessment

An assessment will be carried out within five working days of receipt of all required information. Our preference is for face to face assessment but this can be virtual if needed due to Covid restrictions.
The assessment will be carried out by one of our experienced staff. The assessment will take place in the most convenient location for the individual, which may be their current residence or one of our services. The individuals support and care teams are invited to be present, as we aim to consider the perspectives of all involved with the individuals’ support.
The team will assess the individual using a holistic approach, with an emphasis on understanding the individual beyond their condition. The assessment will look to further identify the individuals mental and physical health support needs, their current situation, potential risk to themselves and others, and explore their ambitions and willingness to engage with recovery.
After the assessment our referrals team will determine suitability and will give feedback to the individual and care team within five working days.

Placement proposal
Our team will provide feedback within five working days from assessment to confirm whether the individual is suitable for placement at our services.
Should the referral be accepted, we will provide a full report to the referring team which will outline key placement information including;
- Summary of assessment
- Support requirements identified and personalised recommendations
- Cost breakdown
- Key information on the transition and placement processes
At this time, we also invite the individuals to visit the service to meet the service manager and their support teams.
Should the referral be determined as unsuitable, we will provide an explanation of why we believe we are not the appropriate solution for their support needs at this time and provide recommendations for alternatives and identify improvements that may improve their suitability for placement in the future.

Placement and funding approval

When a referral is accepted by St Martins for one of our spot purchase placements the proposed placement and funding will need to be approved by the individuals care team, commissioning authority and agreed to by the potential resident.
St Martins will provide support for our referring partners during this process and assist wherever needed, to ensure the approval process is efficient, timely and most importantly, that the individual receives the best outcome possible for their situation.

Transitioning into a St Martins service

Once the placement and funding has been approved we continue to liaise with the individuals care team and current accommodation provider to commence the individuals transition into our service.
Until the person moves into the service we will invite them to come and spend time at the service and get to know the team and other residents.
After move-in our staff will undertake placement reviews at 6 weeks, 12 weeks and 6 months. This allows for reflection on the transition, to assess how the individual is settling into their new home and determine what adjustments may be needed.
